Request Time OffSick and Vacation Requests
Requests should be submitted a minimum of 2-weeks prior to the first date you will be absent from work, or as soon as reasonably practicable after illness or sick time is taken. Please Note: a PTO Form for Sick Time will NOT be considered a call out - please notify your Supervisor if you will be absent from work unexpectedly, in accordance with the Handbook's Call-Out Procedure.
