Section 1: Employment Verification (Form I-9)

This section will focus on the completion of the Employment Eligibility Verification, or Form I-9, that is required by the USCIS.

Items Needed:

• Employee Documents
  • 1 List A OR
  • 1 List B AND 1 List C 
• Employee Personal Information (Name, Address, etc.)

Please note: the information on the I-9 MUST Match any documents provided with the I-9. This information must also be reflected on the PAF, and any other forms submitted. Partial/incomplete names, nicknames, etc, will result in the rejection of the paperwork.

Section 2: Onboarding Paperwork

This section will focus on the completion of Employee Information, including the PAF, and the Emergency Contact Form.

Items Needed:

• Personal Information (DOB, Address, etc.)
• Full Color scan of SSN Card (Do Not Include other I-9 Documents in this section or paperwork will be rejected)
• Emergency Contact Information
• Copy of Bank Verification Document(s)
• Information on Dependents and Taxability
